Plan pays you directly or pays dentist!

No charge for In-Network Oral Exams, Routine Semi-Annual Cleanings, Simple Fillings, Simple Extractions, Topical Sealant, Fluoride Treatment and X-Rays.

Basic services are payable  up to 80% In-Network.

Major services are payable at 50% In-network.

Services include Orthodontic Treatment covered at 50%.

Benefits are payable up to 80% for Out-of-Network services.

Small Annual Deductible.
